If you plan to apply for financial aid in addition to your TOPS, then you need to fill out this form.

COUNSELORS RECEIVE A WEEKLY REPORT OF WHO HAS FILLED OUT THE FAFSA. 

If your student will not need additional financial assistance/student loans in addition to TOPS, you may fill out the TOPS online application instead of the FAFSA.

YOU MUST PRINT THE CONFIRMATION PAGE AND BRING A COPY TO YOUR COUNSELOR. 

If your student will not be attending college, and therefore will not need TOPS tuition or additional student loans, you may fill out a nonparticipation waiver instead of FAFSA or TOPS online application.

THIS FORM MUST BE PRINTED, SIGNED AND TURNED IN TO YOUR COUNSELOR.
